Hormones and the pelvic floor

The pelvic floor is filled by muscles that stretch over the pelvic bone.

The role of these muscles is to support your pelvic organs like the uterus, ovaries, bowel, and bladder.

The pelvic floor changes dramatically during the menstrual cycle, pregnancy, perimenopause, and menopause.

Hormones, especially estrogen, are partially responsible for these changes.

The vagina, bladder trigone, and pelvic floor muscles are rich in estrogen receptors.

After menopause, estrogen reduces. These tissues may become atrophied. Vaginal atrophy causes dryness and painful sex. Bladder atrophy causes leaky bladder (urinary incontinence). The pelvic floor muscle atrophy causes prolapse of the bladder and/or uterus.
 
When prolapse happens, your bladder can slip down lower than usual and bulge into your vagina. See the image below:

Painful sex and leaky bladder are obvious, but how do you know if you have a dropped bladder?

Seven warning signs of a dropped bladder

1) Pelvic pain

A dropped bladder causes pressure on the vaginal wall. This can cause pain, especially when you are lifting, coughing, or straining.

Pain can be sharp, sudden, dull, or continuous.

You may have a consistent, heavy feeling in the pelvis. The pain may intensify during walking or standing.

You may feel an obstruction in the area or may even feel the bladder bulging inside the vaginal tract.

2) Difficulty urinating

You may have difficulty to urinate, have painful urination, or feel you need to urinate more.

When the difficulty increases, you may not be able to urinate at all.

This can become a medical emergency. Doctors can determine if the cause is bladder drop.

3) Stress incontinence

Stress incontinence is the involuntary urine leak. It often happens during physical activity. These include running, jumping, heavy lifting, or when sneezing and coughing.
 
The leakage may only happen when the bladder is full.
 
Weak pelvic floor muscles or the urinary sphincter may cause stress incontinence.
 
A dropped bladder can increase pressure on these structures and cause urine leak. Incontinence can cause embarrassment and reduced physical activities.

4) Urinary retention

The urinary bladder has two jobs. It stores urine and drains it.

There are two circular muscles that surround and enclose the urethra. These are called sphincters.

To empty the bladder, the sphincter muscles and the bladder muscles need to work together. First the sphincter muscles open and then the bladder contracts to empty the urine.

When the bladder is healthy and functioning properly, the urinary bladder empties completely.

A healthy nervous system controls proper bladder functioning.

The brain sends the signal to urinate through the nervous system. These signals allow the sphincter and bladder muscles to open and close.

A dropped bladder may press down on the tissue and nerves. This makes it difficult to completely empty the bladder.

Due to incomplete emptying, you may urinate more than once to empty your bladder. This may need repeated trips to the bathroom. You may feel that you have not emptied completely, even after a full urination. The cause may be a dropped bladder.
 
Additionally, a part of the urine in the bladder falls below the opening of the urethra due to bladder drop. This part of urine does not flush out. The retained urine may cause bacteria to grow in the bladder. This results in smelly urine and frequent UTIs.

5) Frequent bladder infections

When you experience frequent bladder infections, it may be an sign of a dropped bladder.
A bladder infection occurs when bacteria enter the urethra and reach the bladder.
Once in the bladder, bacteria multiply and colonize, causing infection.
When the bladder is healthy, bacteria that travel into the urethra are washed out by urine.
But when you have a dropped bladder, you may have difficulty to empty the bladder. This can result in urinary retention.
This means some urine stays inside of the bladder. When old urine stays in the bladder, bacteria can grow and multiply without being flushed out.
Thus, a dropped bladder may result in frequent bladder infections.

 

6) Lower back pain

A dropped bladder may cause persistent lower back pain.

The bladder is suspended in the pelvis by many ligaments and muscles.

A dropped bladder can fall into the vaginal tract. This can cause the woman to have pain or discomfort in the lower back, pelvis, groin, vagina, and lower abdomen.

You may feel the pain as a continuous aching or pulling sensation.

Lower back pain may become worse during sex or menstrual period.

The dropped bladder may push the neighboring tissues and compress their nerves. This may result in pain in the lower back and pelvis.
A full bladder may pushes harder against neighboring tissues. This may cause the muscles in the lower back to become fatigued and sore.
Also, urinary tract infections in a dropped bladder can also cause pain in the lower back.
 

7) No relief after voiding the bladder

When you feel no relief after voiding the bladder, you may have a dropped bladder.

When the bladder drops, a part of the bladder sags to below the opening of the urethra.
This bulge is called cystocele.
Due to gravity and dislocation of the bladder, you cannot empty the entire bladder. The cystocele traps urine.
When a healthy bladder is full of urine, the nerves in the bladder send a signal to the brain. The brain then signals the sphincter to expel the urine. After you pee, the brain turns off urge you feel to urinate.
Yet, when the bladder is dropped, the brain will continue sending signals to the bladder. These signals cause the sensations that urine is still in the bladder.

 

Medications for painful sex, leaky bladder, and bladder drop

Menopause results in estrogen reduction, which in turn causes painful sex, leaky bladder, and bladder drops. The hormone replacement therapy has been used to treat all these 3 conditions.
Estrogen can be administered orally as a pill or topically as a patch or cream.
 

Ingesting estrogen pills increases the risks of breast, ovary, and uterine cancers. It also increases the risk of stroke, heart attack, and pulmonary embolism. Many women receive estrogen topically.
Vaginal estrogen are used by 16% of women receiving postmenopausal estrogen replacement.
Each year in the United States, over two million women use vaginal estrogen.
Before, vaginal estrogens were thought to have only local effects. Yet, studies showed that estrogen delivered vaginally are quickly absorbed into the blood. It exerts significant systemic effects.
In some cases, vaginally absorbed estrogen can be higher than oral ingested one. It avoids the first-pass metabolism encountered by oral ingestion.
Thus, neither orally nor vaginally delivered estrogen is safe. They both have an increased risk for cancer and cardiovascular diseases.
The cost of estrogen replacement therapy is also high. The typical cost averages $325 per month. It can range between $100 and $550 per month depending on the type and dose used.
In addition to increased risk of breast, ovarian, and uterine cancers, estrogen also promotes uterine fibroids, endometriosis, gallbladder stones, pulmonary embolism (clotting disorder), stroke, and heart attack. Many women, especially breast cancer survivors, cannot use estrogen.

A recent study by scientists at Harvard University provided new evidence indicating that estrogen not only promotes but also directly causes breast cancer. This new discovery further confirms the cancer risk associated with estrogen. 

Treatments with pessary and surgery

To correct the dropped bladder, two methods are often used: pessary and surgery.

Pessary is a device placed in the vagina to hold the bladder in place. But wearing a pessary may cause problem like pain and bleeding. Some may even have difficulty urinating. 

Surgery can move the bladder back to its normal position. But its cost may be too high for many patients.

A bladder drop surgery may cost at least $9,000. Robotic-assisted abdominal surgery is more expensive at around $15,000. This can be a major expense if you do not have medical insurance, or your insurance does not cover all the expenses.

Besides to the high cost, surgery may cause trauma and side effects. The bladder drop surgery has potential complications, including incision bleeding, pain, or infection.
The bladder drop repairing surgery has a very high failure rate ranging from 25 to 60 percent. Many patients and doctors find this high failure rate unacceptable. Repairing bladder drop is one of the most challenging surgeries in medicine.

A pelvic mesh is often used in the surgery to fix the the bladder drop. When the surgery fails, the mesh has to be removed by several follow up surgical procedures. As a result, the patient will have to suffer many times of the unfortunate consequences. 

Many complications are associated with the vaginal mesh. Common complications reported by the FDA include:

  • Erosion of the mesh through the vagina
  • Pelvic pain
  • Serious infection
  • Bleeding
  • Pain during sex
  • Perforation of the organ
  • Urinary problems
Erosion of the vaginal mesh can be devastating. It may need several follow-up corrective surgical procedures to repair the damage. Also, patients may have recurrent prolapse, neuromuscular problems, or vaginal scars/narrowing. They may develop emotional problems. These complications can be painful. They also need extra surgery and hospitalization.
 
Due to high failure rates, the FDA has stopped mesh manufacturers to sell these devices.
 
Thus, a safer and more effective solution would be beneficial.

Can you stop bladder leak and bladder drop from happening?

The answer is "Yes."

When I started Silk sensitive I was also weaning off of hormone cream. Also using Balm and then added Sea Buckthorn oil. There have been no adverse side-effects with any NeuEve products. IT REALLY WORKS, EVEN FOR BLADDER PROLAPSE!!!!

Excellent product! I can't say enough about this.

I was diagnosed 2.5 months ago with Bladder Prolapse. My Gynecologist recommended

Estrogen suppositories for 3 months to strengthen the area inside. He said that without that, surgery would surely fail.

I'm very pleased!

I am 57 years old. I am very active. I strength train/lift weights 4 to 5 days a week and ride my own motorcycle, so this "condition" caught me by surprise.
I felt my bladder dropping and this is very disconcerting. I spoke to my doctor about it and got some info about a condition called prolapsed bladder.
Hormones and surgery were my only options, really, and neither was very attractive to me. I decided to try NeuEve Silk after doing some research about this condition. The price is fairly reasonable and all natural ingredients is a major win.

Miracle

I had 8 UTIs in 8 months. My doctor wanted me to use estrogen cream. I did not. After researching I found NeuEve vulva balm cream which appeared to be all natural and same concept as estrogen.

I started using it. No UTIs since starting product.

I developed chronic dry, peeling skin around eyes and nothing worked. Although it isn’t an eye cream, I put this same cream around eyes and they are healed and actually appear to have reduced lines and my eyes look bright.

Perfect alternative

I have only been using this cream for a few months now. I cannot use hormone treatment because of breast cancer. Very happy with this product and being all-natural. Finding relief

Over the past year I had been experiencing frequent urination and itching, as though I had an infection. I went to my gynecologist who advised me that I was experiencing severe vaginal dryness. I tried a few products that only helped for a few days before the problem returned. I searched the web for help and found NeuEve. The reviews were positive so I gave it a try. Yesterday I received my NeuEve Silk and I knew I had to try it at bedtime. I have been in menopause since I was 33 years old and I am 64 now. I had cancer and therefore was afraid to use estrogen cream vaginally which is what doctors have said for years that I need. I was afraid to use it, so I just did not listen.

Fast forward to now and I have been miserable. I have had a dull aching and it feels like when you feel like a UTI is coming on not to mention the awful dryness, so the doctor said I had no choice but to use the estrogen.

I was so miserable I started using it. Taking the instructions out and reading all the warnings freaked me out! So, I got on the internet and prayed there was something else out there to help me, there just had to be!

That is when I stumbled upon the info about NeuEve. I read everything I could find on it and felt like I had nothing to lose because the options were few and far between. I put my first suppository last night and it didn’t take long to melt, and my body absorbed it almost immediately that is just how dry I am “down there”! I ordered these for my mother-in-law and this is what she has to say:

"My urologist told me to get these! I was having constant UTI's for the past few years, I'm 58 and after being diagnosed with breast cancer at 49 and then being told to have my ovaries removed too. I had everything taken out except my vagina. After that, it was much too painful for any of that kind of fun!

I do believe in miracles and maybe your product will make it possible again!!! Your product has made me feel so much better! I do not have the horrible feeling of getting a UTI anymore or any dryness and not one UTI since I started!!! Which is huge!!! So, thank you for caring enough for women's health to make such a wonderful product!!!

This is such a hard subject to discuss no one talks about it and not something I normally would but I think I need to tell you it is working so far! The price is high, but I will just have to save somewhere else!!!

I will try not to go into a lot of detail, but at 61, I was beginning to wonder whether I should once again use an estrogen-based product for vaginal atrophy.

I first tried estrogen when my GYN. recommended it after post-menopause. I used it for about a year and it did work very well, but just did not like using estrogen - I especially do not like that it comes from horse urine. That was about 6 or 7 yrs. ago.

The conversation came up again after a stubborn UTI, then an antibiotic-induced yeast infection. There seems to be some literature suggesting that atrophy and UTI's are connected. But I just do not want to use estrogen - and literature is varied on the opinions - health benefits and risks. I thought I'd give NeuEve a try as I had used another brand of vaginal suppository before - but the other brand had a medicinal smell I didn't like, and it didn't do much to change the atrophy.

This product has impressed me. I also ordered the small jar of cream to put on daily. So far, so good. I have felt much more comfortable. The product is more than I would like to pay, but I ordered my second box. I started with Silk - their recommendation but ordered Silver the second time by accident --because I think the Silk formula was doing its job. But now I will be able to compare anyway. I recommended at least trying it.

My urologist recommends these suppositories. I found them to be most effective. Still use in conjunction with estrogen cream, so cannot say how effective they would be on their own. For now, they meet my immediate needs in the manner that I am using them.

When deciding whether to purchase this product I read through almost all the reviews. Most said either the product worked or did not and no other information is given, so I want to write a review that will hopefully be helpful to others.

I was widowed at a relatively young age and had not had sex for over 12 years. I recently met someone, and we wanted to be intimate. I talked to my doctor and she gave me a prescription for hormone cream. It did not work and having sex was a disaster! It was very painful, I had spotting due to vaginal atrophy, and got a flaming UTI. I did not want to give up so I went online looking for a product that would work for me.

I found the New Eve products and saw that they were available on Amazon, so I tried them. The next time we had sex, there was NO spotting at all which I was thrilled about. The 3rd time there was no spotting, no pain, and no UTI. The product worked for me and it was worth the price. I am not going to go looking for something cheaper because there is no guarantee it will work. I have something that works and I'm happy about that.

It has been 4-1/2 months since I started using NewEve, following the regimen with the cream, Silk, and Silver. Lesson learned: The more advanced the atrophy, the more time and patience it takes to reverse the condition. In fact, at age 76, the results are nothing short of miraculous.

Helped my UTIs

I am a breast cancer survivor with chronic UTIs. I was told that they were caused by vaginal atrophy due to loss of estrogen, but I cannot use any estrogen due to the cancer diagnosis. I tried this product to see if it would help, and to my amazement, it did!

The atrophy was causing inflammation in my urinary tract, and the inflammation was attracting bacteria, generating an infection. I had one almost constantly for 3 years.

Literally the first time I used the product I could see and feel a difference. I have not had a UTI since starting this product. The rest of the benefits of no atrophy are great, but I am so grateful to have found this helpful product for my urinary tract. Hopefully, other sufferers will find the same result.

I have been using the Silk formula for about five months. I had post-menopausal vaginal dryness which was contributing to repeated UTIs. The dry atmosphere promoted bacterial growth. In addition, I had extreme vulvar pain (vulvodynia) and every remedy I tried felt as though I was applying acid.

I was desperate after three rounds of antibiotics in four months and still had horrible burning pain. My doctor prescribed hormone cream but I was reluctant to use it since there is a history of breast cancer in my family.

While desperately searching online (sitting on a cold compress at the time) I came across NeuEve and ordered immediately, both the silk formula and the cream to start with. Thank you so much and I am sure you've heard it before, but this is such an answer to prayer!!!! Thank you.

I am just turning 50 this year but very healthy, young, and live very organic and holistically. I was devastated to start having painful sex issues thinking there was nothing all-natural to help.

To find your product was a little miracle for me! Also, I think it took care of the other little issue I had with small tiny but growing white bumps on the outside of the Urethra and the Urologist said it was an Estrogen problem. THANK YOU!!

    Thank you so much for reaching out, Kathy Pollitt. The bladder is a hollow organ in the pelvis that stores urine. In women, the front wall of the vagina supports the bladder. This wall can weaken or loosen with age. Significant bodily stress such as childbirth can also damage this part of the vaginal wall. If it deteriorates enough, the bladder can prolapse, meaning it is no longer supported and descends into the vagina. This may trigger problems such as urinary difficulties, discomfort, and stress incontinence (urine leakage caused by sneezing, coughing, and exertion, for example). Thus, women may develop bladder prolapse even with a normal level of estrogen.

    You can use NeuEve Silver if you are under 50 or Silk if you are over 50 and balm cream to help strengthen the vaginal wall. After the vaginal wall is strengthened, the bladder will gain more support and regain its upright position.

    Well, I am 69, so I figured the ‘under 70’ applied to me. I ordered the Gold and afterwards read everything on the matter. I don’t think Gold should have been my first choice but, I tolerated it well with just a smidgen of irritation the first insertion. I did my fourth treatment last night and much to my husbands surprise and my shock this afternoon, I am almost back to me. We have always been very active and I didn’t have any problems until I hit over 68. I just placed the recurring monthly order feature. If you are on the fence, jump.

    I would like to know if it’s too late for treatment for a stage 3 cystocele or bladder prolapse .I am 73 and have had 5 children.I am considering a colpocleisis Surgical Vaginal closing.as a last result.I have issues with incontinance and some vaginal atrophy. I am willing to try anything before making final DECESION about the surgery.can you offer any information or advice for which products for me or if they would help me in such an advanced stage.
